Purpose:

To support adults with developmental/learning disabilities in developing and maintaining independent living skills.  

Reports to:

Director of Transitional Living/Director of Community Living

Qualifications:

  1. B.A. in Special Education, Rehabilitation Counseling or related field preferred and/or minimum of two years’ experience working with adults with learning/developmental disabilities required, preferably in a community setting.  

  2. Clean driving record.

  3. Clear criminal background.

  4. First Aid certification.

  5. Willingness to work flexible hours to meet resident needs. 

Responsibilities:

Responsible for assessing residents interested in living independently and developing Individual Support Plans every six months, regularly monitoring goal progress, and updating resident records.

  1. Provide updated resident information on MF Face Sheets to all staff regarding schedule changes, med changes, address and phone number changes, monthly and/or as needed. 

  2. Provide instruction and support around all areas of daily living, including, but not limited to: meal preparation, shopping, hygiene, personal care, budgeting, safety awareness and maintaining a clean and healthy living environment. 

  3. Monitor and support residents taking medications.

  4. Maintain regular, ongoing contact with each resident’s family or guardian with regard to goals, progress, strengths and/or emergencies. 

  5. Assist residents in coordination of all medical/dental/psychological appointments documenting all appointments on the  MF Client Medical/Dental record. 

 Help residents develop skills needed for independent living.

  1. Teach rights and the development of advocacy skills.

  2. Responsible for maintaining professional conduct with residents.

  3. Responsible for maintaining confidentiality.

  4. Assist residents in developing natural networks of support.

  5. Provide training and support in areas that enable residents to maintain their own homes in the community of their choice.  

  6. Assist with organization of resident’s personal belongings. 

  7. Assist residents in managing personal affairs.  

Collaborate with Case Management team.

  1. Meet with supervisor as scheduled to review cases. 

  2. May teach, assist and/or facilitate classes, dinner groups, safety classes, fire drills and special events/parties.

  3. Transport residents to appointments, social activities, meetings.

  4. Attend and participate in all required staff meetings, training and professional development opportunities. 

Other duties as assigned by supervisor which may include:

  1. Maintain and update chore/cooking schedules

  2. Schedule and perform monthly safety meetings and fire drills

  3. Schedule and perform house/apartment meetings to troubleshoot issues with residents.
